29 CFR 1926.1412(d)(1): Visual inspections were not performed by a competent person prior to each shift for apparent deficiencies of the equipment that was used:    On or about July 29, 2014, employees were exposed to struck-by hazards, when a daily inspection of the Grove RT550E serial #223147 was not conducted, and having discrepancies such as: loading chart not posted inside the cab and a fire extinguisher was missing, while the crane had lifted and was suspending a bridge steel beam.

29 CFR 1926.1412(e)(3)(i): The following information must be documented and maintained by the employer that conducts the inspection: (A) The items checked and the results of the inspection. (B) The name and signature of the person who conducted the inspection and the date.     On or about July 29, 2014, employees were exposed to struck-by hazards, when using a Grove mobile crane model #RT530E with serial #223147 where the inspection information was not documented and maintained as required, while lifting and suspending a bridge steel beam.
