29 CFR 1926.501(b)(7)(ii): Each employee at the edge of a well, pit, shaft, or similar excavation 6 feet (1.8 m) or more in depth was not protected from falling by guardrail systems, fences, barricades, or covers: Worksite located at 1221 Ed Noble Parkway. Norman, OK 73072. On or about July 29, 2014, an employee was observed working at the south side edge of an unprotected trench. The employee was exposed to a fall hazard of approximately 20 feet to the bottom of the trench.

29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: North side of trench. Worksite located at 1221 Ed Noble Parkway. Norman, OK 73072. On or about July 29, 2014, employees were observed working inside a trench and were not protected from loose rock or soil where the edge of the protective barrier did not adequately extend above the sloped soil along one entire length of the barrier. The employees were exposed to a struck-by hazard from falling rock or soil.
