29 CFR 1910.212(a): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about 07/28/2014 - Machine shop, employees were exposed to unguarded saw blades of up to nine and one-half inches in length while using horizontal table saws such as the W.F. Wells 2-Post Saw, Model F16 and the W.F. Wells 2-Post Saw, Model W9. (b) On or about 07/28/2014 - Machine shop, CNC operators were exposed to struck-by injuries from rotating parts and/or flying chips while operating the Haas CNC TM-2 which would continue to operate when the doors were opened.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) On or about 07/28/2014 - Machine shop, a lockout / tagout program was not established for contractor(s) hired to perform servicing and maintenance on machines such as, but not limited to, a Wotan Horizontal Mill, W.F. Wells 2-Post Saws, Dainichi Techno Turn, Fanuc 21T, Dainichi Flash Turn, Fanuc 21T, Haas CNC TM-2, Haas CNC VF-5, Jenfab Parts Washer, Takisawa TT-350G Twin Spindle Lathe, and a Quincy 30HP Air Compressor.
