29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about July 25, 2014, on the campus of Tulsa University, an employee was allowed to work on a steep pitched roof without fall protection, exposing the employee to the hazard of falls of approximately 30 feet.

29 CFR 1926.1053(b)(1): A portable ladder used for access to an upper landing surface did not have the ladder side rails extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support. On or about July 25, 2014, on the campus of Tulsa University, employees were allowed to use a ladder that did not extended 3 feet above the point of contact or secured to the structure to ensure safe access/egress, exposing employees to the hazard of falls.
