29 CFR 1926.453 (b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. On or about June 18, 2014, the employer did not ensure employees working inside of a JLG aerial lift were in use of fall protection, exposing the employees to a fall hazard.
