29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards: Note: Toe boards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.    On or about July 30, 2014 and times prior thereto, employees of Newton Well Service, LLC performed service on the Tatum B-1 oil well with the use of a Franks Explorer II workover rig located at or near latitude North 33.317417, and longitude West 92.835583 in Mount Holly, Arkansas.  The rig platform was not guarded with open sides to the ground level below, exposing employees to falls greater than 4 feet.

29 CFR 1910.27(d)(5): "Ladder safety devices." Ladder safety devices may be used on tower, water tank, and chimney ladders over 20 feet in unbroken length in lieu of cage protection. No landing platform is required in these cases. All ladder safety devices such as those that incorporate lifebelts, friction brakes, and sliding attachments shall meet the design requirements of the ladders which they serve.    On or about July 30, 2014 and times prior thereto, employees of Newton Well Service, Inc. performed service of the Tatum B-1 oil well with the use of a Franks Explorer II workover rig at or near latitude North 33.317417 and longitude West 92.835583 in Mount Holly, Arkansas. The employer had not provided employees working in the derrick and substructure with a climb assist device for the ladder used, exposing employees to fall hazards when accessing platforms greater than 20 feet from ground level.

29 CFR 1910.132(c): Personal protective equipment was not of safe design and construction for the work to be performed:    On or about July 30, 2014 and times prior thereto, employees of Newton Well Service, LLC, performed service on Tatum B-1 oil well with the use of a Franks Explorer II workover rig located at or near latitude North 33.317417 and longitude West 92.835583.  The employer had provided personal protective equipment which was not of a safe design for the work performed in the following instances:    a. The employees working from the derrick had been provided and were required to wear a body belt as a means of fall protection, which was not designed or of safe construction to prevent from falls.    b. The employees working from the derrick were provided and required to use 8" flexible steel tail ropes as a means for fall arrest, which were not of safe design and construction to prevent from falls.

The employees performed service operations which were not in near proximity to an infirmary, clinic, or hospital and the employer did not ensure there was a person onsite trained to render first aid.
