29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   The employer is failing to protect employees from potential fall hazards associated with re-roofing an existing residential home where the ground to eave is greater than 6 feet and the roof pitch is more than 4:12.  The most recent example of this was found on the job site located at 1095 16th Street, Blair, NE 68008 where the employer has unprotected employees re-roofing an existing residential home without the use of fall protection.

The most recent example of this was found on the job site located at 1095 16th Street, Blair, NE 68008 where the employer has untrained employees re-roofing an existing residential home without the use of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer is failing to protect employees from potential fall hazards associated with using ladders as a means of access and egress from elevated work surfaces where the ground to eave is greater than 6 feet and the roof pitch is more than 4:12.  The most recent example of this was found on the job site located at 1095 16th Street, Blair, NE 68008 where the employer has employees using an aluminum ladder as a means of access to the roof of an existing residential home without extending the ladder a minimum of three feet over the upper landing surface.
