29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     The employer is failing to provide training to employees on the potential fall hazards associated with re-roofing an existing residential home where the ground to eave is greater than 6 feet and the roof pitch is more than 4:12.  The most recent example of this was found on the job site located at 1466 Park St., Blair, NE 68008 where the employer has untrained employees re-roofing an existing residential home without the use of fall protection.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:     The employer is failing to protect employees from potential fall hazards associated with using ladders as a means of access and egress from elevated work surfaces where the ground to eave is greater than 6 feet and the roof pitch is more than 4:12.  The most recent example of this was found on the job site located at 1466 Park St., Blair, NE 68008 where the employer has taken a 25 aluminum extension ladder and separated the upper portion from the lower portion and has employees using both sections to access roof of an existing residential home.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      The employer is failing to protect employees from potential fall hazards associated with re-roofing an existing residential home where the ground to eave is greater than 6 feet and the roof pitch is more than 4:12.  The most recent example of this was found on the job site located at 1466 Park St., Blair, NE 68008 where the employer has employees re-roofing an existing residential home without using any fall protection.        Geraldo Navarro was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 314061953 , citation number 1, item number 1 and was affirmed as a final order on 9/6/2011, with respect to a workplace located at 15610 Chasemore Dr. Plattsmouth, NE 68048 and became a Final Order effective November 11, 2011.
