29 CFR 1910.334(c)(1): Unqualified persons performed testing work on electric circuits or equipment:    a. High Voltage Cage, Lab 2- An employee was not protected from electric shock hazards where they were allowed to do voltage testing work without being adequately trained by the employer. On or about July 29, 2014 an unqualified employee sustained an electric shock while voltage testing a power supply producing up to approximately 8900 volts DC.

29 CFR 1910.334(c)(2): Test instruments and equipment and all associated test leads, cables, power cords, probes, and connectors were not visually inspected for external defects and damage before the electric equipment was used:    a. High Voltage Cage, Lab 2- Employees were not protected from electric shock hazards where the employer failed to ensure that a CPS HPV-250 high voltage probe was inspected for external defects and damage before each use. On or about July 29, 2014 an unqualified employee sustained an electric shock while voltage testing a power supply producing up to approximately 8900 volts DC. The employee was using the CPS HPV-250 high voltage probe that was modified by the employer by adding a lead that connects the probe tip to the test point. The employee contacted the lead with their hand and received an electric shock.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:    a. High Voltage Cage, Lab 2- Employees were not protected from electric shock hazards where the employer failed to require the wearing of insulated gloves when using a CPS HPV-250 high voltage probe. On or about July 29, 2014 an unqualified employee sustained an electric shock while voltage testing a power supply producing up to approximately 8900 volts DC. The employee was using the CPS HPV-250 high voltage probe that was modified by the employer by adding a lead that connects the probe tip to the test point. The employee contacted the lead with their hand and received an electric shock.
