29 CFR 1910.1001(j)(4)(iv): 1910.1001(j)(v): Signs identifying the presence, location and work practices to not disturb the ACM and/or PACM contained within the area were not posted at the entrance where employees were reasonably expected to enter: (a) On or about May 20, 2014, in the facility, engineering, maintenance and housekeeping employees were required to perform activities in area which contained asbestos containing materials, analyzed to contain 8% chrysotile asbestos. Signs identifying the presence, location and work practices to not disturb the ACM and/or PACM contained within the area were not posted at the entrance where employees were reasonably expected to enter, thereby, exposing employees to the hazards associated with asbestos.

29 CFR 1910.1001(j)(7)(iv): The employer did not provide to all employees who perform housekeeping work in areas where ACM and/or PACM is present, an asbestos awareness training course covering the health effects of asbestos; and/or locations of ACM and/or recognition of ACM and PACM damage and deterioration and/or requirements in this standard related to housekeeping, and/or proper response to fiber release episodes: (a) On or about May 20, 2014, in the facility, engineering, maintenance and housekeeping employees were required to perform activities in area which contained asbestos containing materials, analyzed to contain 8% chrysotile asbestos. The employees were not provided with asbestos awareness training, thereby, exposing employees to the hazards associated with asbestos.

Surfaces of the locker room(s) and resident personal belongings were not maintained as free as practicable of ACM surfacing (fireproofing) waste and debris and accompanying dust, thereby, exposing employees to the hazards associated with asbestos.
