OSH ACT of 1970 Section (5)(a)(1):     OSH Act of 1970 Section 5(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to amputation hazards:    On or about May 27, 2014, an employee who had not been properly trained was exposed to amputation hazards when he accessed the tube cutting machine through the middle door while the machine was running to adjust cut depth and/or clear jams.    Among other methods, one feasible and acceptable abatement method to correct theses hazards is:  1.  Conduct a job hazard analysis (JHA) to identify all hazards posed by the operation of the tube cutter and the methods to prevent employee exposure to the hazards;  2.  Establish a machine operator training program, with review and approval of training content by a management official.  The manufacturer's operating manual and JHA shall be used during the training to ensure that employees understand the potential hazards and the mechanisms in place for their protection;   3.  Train machine operators in the safe operation of the equipment upon initial assignment, after any changes are made to the equipment or job tasks, or when employee action demonstrates that there is a knowledge deficiency/lack of understanding.  Document completion of training activities through written certification that includes the instructors name, date of training, participant names, topics discussed, and signatures of the instructor and a management official;   4.  Evaluate training effectiveness through management observation of the employees during normal operation and during deviations from normal operation.  The evaluations shall be performed upon initial assignment and at least annually thereafter;   5.  Prohibit operators from opening the doors to the tube cutter or bypassing other safeguarding devices except as designated by written operating procedures; and  6.  Communicate the above work rule during initial training and refresher training to be conducted as needed; and  7.  Conduct regular safety inspections, in addition to inspections conducted by the operator, of the tube cutter to ensure that all safety devices and mechanisms are in place and functioning as intended.  Document the inspections to include the date of the inspection, name of the inspector, deficiencies found during the inspection, corrective action dates, and certification by a management official that the equipment is safe to operate.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:    On or about May 27, 2014, employees were exposed to amputation hazards when employees were not trained to perform lockout/tagout when bypassing the door (guard) on the tube cutter.
