29 CFR 1903.19(c)(1): Within 10 calendar days after the abatement date, the employer must certify to OSHA (the Agency) that each cited violation has been abated, except as provided in paragraph (c)(2) of this section.  The employer failed to provide certification that the following cited items were corrected:  a.  Employees exposed to fall hazards were not trained on fall protection procedures.  b.  Employees accessing a roof with an extension ladder that did not extend 3 feet above the landing surface nor was it tied off.  c.  Employees were not trained to recognize hazards associated with extension ladders.

29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  The employer did not submit information documenting that employees engaged in roofing activities at heights greater than 6 feet were provided fall protection.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     On or about July 23, 2014, employees were accessing a roof with an extension ladder that did not extend 3 feet above the landing surface nor was it tied off.      ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM IN ACCORDANCE WITH REQUIREMENTS OF 29 CFR 1903.19(C).
