29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:    a-25 Jacobsen Lane, Normandy Beach NJ: Company employees, not supplied with any fall protection, were exposed to fall hazards in excess of 14 feet to the ground surface below when they were not provided with direct access to the ladder jack's walking/working surface.  Condition observed on or about 07/29/14.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  a-25 Jacobsen Lane, Normandy Beach NJ: Company employees, not supplied with any fall protection, were exposed to fall hazards in excess of 14 feet to the ground surface below when performing roof shingle installation, and other related activities, from a ladder jack scaffold.  Condition observed on or about 07/29/14.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause them to lose balance and fall:    a-25 Jacobsen Lane, Normandy Beach NJ: A company employee was exposed to fall hazards of approximately 21 feet 4 inches to the ground surface below when descending an aluminum extension ladder while carrying equipment that included a pneumatic nailer.  In doing such the employee was unable to maintain three points of contact therefore, exposing themselves to conditions where they could lose their balance and fall.  Condition observed on or about 07/29/14.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:    a-25 Jacobsen Lane, Normandy Beach NJ: Company employees, wearing full body harnesses that were not attached to lanyards, were exposed to fall hazards of approximately 21 feet 4 inches to the ground surface below when performing roof shingle installation, and other related activities, from a roof with a pitch of approximately 12 on 12.  Condition observed on or about 07/29/14.    U.S. Contractor, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 924687, citation number1, item number 1, and was affirmed as a final order on 9/30/13, with respect to a workplace located at 989 East Avenue, Mantoloking, NJ.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)    a-25 Jacobsen Lane, Normandy Beach NJ: Company employees, wearing full body harnesses that were not attached to lanyards, were exposed to fall hazards of approximately 21 feet 4 inches to the ground surface below when performing roof shingle installation, and other related activities, from a roof with a pitch of approximately 12 on 12.  These employee had not received fall protection training from the company.  Condition observed on or about 07/29/14.    U.S. Contractor, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(2), which was contained in OSHA inspection number 924687, citation number1, item number 2, and was affirmed as a final order on 9/30/13, with respect to a workplace located at 989 East Avenue, Mantoloking, NJ.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
