29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions: (a) New Cafeteria - On or about July 23, 2014 an employee was exposed to struck-by and crushing hazards while operating a Gradall Rough terrain forklift with a rollover protective structure. Employee was not wearing the seatbelt installed in the vehicle.

(b) Outside the New Classrooms - On or about July 23, 2014 employees were exposed to impalement hazards while working near or walking near steel reinforcing rods that were not capped. (c) New Cafeteria, Perimeter Foundation - On or about July 23, 2014 employees were exposed to impalement hazards while working near or walking near steel reinforcing rods were not capped.
