29 CFR 1910.134(c)(2)(ii):  The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored and maintained so as not to create a health hazard to the user:    1) On July 31, 2014, and at prior times, the employer had not ensured that paint department employees who wear 1/2 mask respirators were medically able to use respirators.

29 CFR 1910.1025(h)(1):  All surfaces were not maintained as free as practicable of accumulations of lead:    1) On July 31, 2014, a table in the paint room where paint is dispensed from cans was contaminated with lead at a level of approximately 679 ug/100cm2.    2) On July 31, 2014, the table where paint is applied was contaminated with material that contained 1.02% lead.

29 CFR 1910.305(g)(1)(iii):  Flexible cords and cables were used for purposes prohibited by subparagraphs (A) through (E) of this paragraph:                                                                                 1) On July 31, 2014, and at prior times, employees in the maintenance department were exposed to shock hazards where a flexible cord was used in the place of permanent wiring, where the cord ran from a junction box to an outlet box.  Prohibited by paragraph (A) of this standard.
