On or about August 29, 2014, at the front of the building, a written hazard communication program was not implemented and maintained at the workplace where employees handled hazardous chemicals including but not limited to crystalline silica present in brick and masonry cement.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    a. On or about August 29, 2014, at front of building, a yellow extension cord lacked strain relief in that the outer jacket was pulled away from the attachment plug exposing the covered conductors which exposes employees to electrical hazards.
