29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a) The employer failed to ensure that employees conducting insulation work on a residential building were protected from fall hazards, through the use of appropriate fall protection.  This exposed employees to fall hazards of approximately 11.8 feet and 24 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) The employer failed to ensure that the ladder utilized to access the overhang was extended at least 3 feet above the landing.  This exposed employees to a fall hazard of approximately 11.8 feet.
