29 CFR 1910.305(g)(1)(iv): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used:  29 CFR 1910.305(g)(1)(iv)(B): Where run through holes in walls, ceilings, or floors;    a. In the Drivers break room the flexible cord for the TV was run through the ceiling tile. Employees were exposed to a fire hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.    a. The flexible cord for the Miller CS-300 Arc Welder was not provided strain relief at the plug. Employees were exposed to 240Volt 32 Amp electrical current.

29 CFR 1910.334(a)(2)(ii): If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made.    a. At the fuel island the flexible cord for the pump that handles the Diesel exhaust fluid was damaged and the employer wrapped it in electrical tape. The employees were exposed to contact with electrical current.

Equipment, wiring methods, and installations of equipment in hazardous (classified) locations shall be intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location. Requirements for each of these options are as follows:  29 CFR 1910.307(c)(2): Approved for the hazardous (classified) location.  a. In the garage pit 5 the employer had a non rated switch wired to a flexible cord that powered a sump pump. The switch was not rated for Class 1 hazardous location. The employees were exposed to a fire/explosion hazard.  b. In the garage pit 6 the employer had a non rated switch wired to a flexible cord that powered a sump pump. The switch was not rated for Class 1 hazardous location. The employees were exposed to a fire/explosion hazard.
