(a) PENDU Manufacturing 300 Log - The employer did enter complete descriptions in column F of the Log of Work Related Injuries and Illnesses (OSHA 300) of the injury or illness, parts of body affected and object that directly injured each employee who sustained a work related injury or illness. (b) PENDU Manufacturing 300 Log - The employer did not classify each case with complete entries in columns G through L of the Log of Work Related Injuries and Illnesses (OSHA 300). (c) PENDU Manufacturing 300A Log - The employer did not enter the correct actual total hours worked by all employees last year on the Summary of Work Related Injuries and Illnesses (OSHA 300A). No abatement certification or documentation required.
