29 CFR 1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use: LOCATION: 18th floor renovation, 40 west 57th Street, NY NY 10019, by electrical panel and by bathroom on Northwest corner of project. On or About July 31, 2014 A). Employees were utilizing a Makita brand 4" grinder which was not protected or fitted with the designed wheel guards in place.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: LOCATION: 40 West 57th Street, New York, NY 10019, 18th Floor North side by bathroom. On or About July 31, 2014, (a)Employees installing tile in the bathroom on the north side of the project were exposed to electrical shock hazards because the ground pin had been removed/broken off on the flexible extension cords being used to power thier grinder.
