29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.     a. At the worksite where employees were conducting demolition of a building, the employer failed to initiate and maintain an effective safety and health program to protect employees from injury and illness.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a. On the roof of the building where employees were removing the wooden decking in the demolition process, the employees were not protected from falls, thereby exposing employees to 20 foot fall hazards.

29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey shall be made, by a competent person, of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure. Any adjacent structure where employees may be exposed shall also be similarly checked. The employer shall have in writing evidence that such a survey has been performed.    a. At the demolition site where employees were removing building materials to include structural members, an engineering survey was not completed, thereby exposing employees to struck-by or crushed-by hazards.
