On or about July 31, 2014, in the weld shop, the oxygen gas cylinders, acetylene gas cylinders and 3 non-flammable mix gas cylinders were not secured to prevent them from being knocked over, exposing employees to struck by hazards.

On or about July 31, 2014, at the welding, there were three propane fuel cylinders obstructing the exit door, exposing employees to tripping hazards.
