29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:   a.      On or about July 31, 2014, in the weld shop, the oxygen gas cylinders, acetylene gas cylinders and 3 non-flammable mix gas cylinders  were not secured to prevent them from being knocked over, exposing employees to struck by hazards.

29 CFR 1910.253(b)(2)(ii): Inside of buildings, cylinders were not stored in a well-protected, well-ventilated, dry location, at least 20 feet (6.1 m) from highly combustible materials such as oil or excelsior.  Assigned storage spaces for cylinders was not located away from elevators, stairs, or gangways where cylinders could not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons:  a.      On or about July 31, 2014, at the welding, there were three propane fuel cylinders obstructing the exit door, exposing employees to tripping hazards.
