29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  On or about July 29, 2014, and at times prior thereto, the employer did not maintain the work area below a scaffold free from debris such as, but not limited to, road signs, buckets, lumber, and scaffold bracing, exposing employees to the hazards of slip, trips, and falls.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a)    On or about July 29, 2014, an employee performing housekeeping activities was working on a balcony edge with an unprotected side in residential construction without the use of fall protection equipment or an alternative fall protection plan, exposing employees to the hazards of falls greater than 6 feet or more to the ground.   b)    On or about July 31, 2014, employees performing plastering operations were working on a balcony edge with an unprotected side in residential construction without the use of fall protection equipment or an alternative fall protection plan, exposing employees to the hazards of falls greater than 6 feet or more to the ground.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about July 29, 2014, and at times prior thereto, employees were exposed to a fall hazard greater than 6 feet to the ground while performing housekeeping and plastering operations.
