29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) The Kuraki CNC Horizontal Boring/Milling Machine H13-25P (H13-3217) had inadequate guarding for the moving table and headstock exposing employees to struck-by and caught in hazards. Photo 19-21 (b) The Kuraki CNC Horizontal Boring/Milling Machine (KBT-3578) had inadequate guarding for the moving table and headstock exposing employees to struck-by and caught-in hazards. Photo 43-44 (c) The Kuraki CNC Horizontal Boring/Milling Machine KBT-11WDX (KBT-3244) had inadequate guarding for the moving table and headstock exposing employees to struck-by and caught-in hazards. Photo 11-12 (m) The OKK MVC 660 vertical machining center (MCV-404) had inadequate guarding in that there were movable doors allowed employees to access the point of operation exposing employees to struck-by hazards. The movable doors and guard also allowed employees to reach over the top. Photo 13-14 (n) The Kuraki CNC Vertical Machining Center KV-1000 (K-9676) had inadequate guarding in that there were movable doors allowed employees to access the point of operation exposing employees to struck-by hazards. The movable doors and guard also allowed employees to reach over the top. Photo 15-16 (o) The OKK MCV660 (MCV-190) vertical machining center had inadequate guarding in that there were movable doors allowed employees to access the point of operation exposing employees to struck-by hazards. The movable doors and guard also allowed employees to reach over the top. Photo 17-18 (p) The HNK Machine Tool (HB30817) had the rear door open due to the part being machine did not fit inside. This allowed employees to access the point of operation exposing employees to struck-by hazards. (q) The Kuraki AKB-13 (AKB-6507) horizontal boring/milling machine had the back doors taken off because the parts were too big and when the table rotated would stick the doors. This allowed employees to access the point of operation exposing employees to struck-by hazards.

29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: (a) The employer's Lockout/Tagout procedures for the horizontal boring bar machines did not include the steps necessary to verify the equipment was properly de-energized.
