29 CFR 1910.159(c)(10): The minimum vertical clearance of 18 inches between automatic sprinkler systems sprinklers and the material below was not maintained:  Automatic water sprinkler heads were obstructed in two different location, exposing employees to burns from fire.  1)  In the Pre-Press area, a water sprinkler head mounted on the stairway leading to the overhead plate storage area was blocked by a stack of papers resting on a file cabinet located approximately 5 inches below the sprinkler head.  2) In the Overhead Plate Storage Area, cardboard boxes, which contained printing plates were stored within approximately 10 inches from water sprinkler heads.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  In the break room, a flexible cords, which was plugged into a 110 VAC electrical outlet, was being used to power a multi-receptacle extension cord, which had a flexible cable running to a cabinet mounted electrical outlet, exposing employees to a fire hazard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  In the forklift charging area, the Powerhouse Charger has cabling that had pulled from the attachment plug, exposing bare copper on one of the leads, resulting in an electrical shock hazard for employees.
