29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of 29 CFR 1926.501:    On or about 07/25/2014 during roofing activities, employee(s) installing asphalt shingles on an apartment building located at 3485 E. Prescott Cir. in Cuyahoga Falls, OH without fall protection were exposed to a fall hazard from a height of approximately 15(+) feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards:     On and before 07/25/2014, at the address of 3485 E. Prescott Cir., Cuyahoga Falls, OH, the employer failed to ensure that employees completing roofing activities were trained in the appropriate use of fall protection. Equipment includes, but is not limited to, personal fall arrest systems or devices.
