29 CFR 1926.1434(a)(1): Modifications or additions which affect the capacity or safe operation of the equipment were made without manufacturer review and approval: Along the south east side of the highway construction site, on or about July 31, 2014, and at times prior thereto employees were exposed to struck-by hazards when a Grove hydraulic crane model RT600E was being operated doing concrete pouring in which the following modifications were done on the original equipment without having manufacturer's review and approval: a) Welding work done on all outrigger floats with material such as but not limited to unrated/unidentified bolt, nut fasteners and rebar material. b) The hooks on the block and the ball assembly were repaired with unrated/unidentified bolt, nut fasteners and rebar material rendering the latch locking mechanism of the hooks inadequate.
