29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6). Jobsite: Employees were working from pump jack scaffolding that was erected less than 3 feet from insulated power lines where the voltage was approximately 240 volts. The power lines were resting on the pump jack scaffolding and was approximately 12-18 inches from the scaffolding along the length of the scaffolding.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. Jobsite: Pump jack scaffolding had been erected without bracing at the bottom or other points as required by manufacturer's instruction exposing employees to fall, struck-by, and crushed-by hazards.

29 CFR 1926.453(a)(2): Field modification of the aerial lift was not certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANSI A92.2-1969: Jobsite: Employees were using a Genie Model S-65, Serial S6013-25189 telescopic boom lift where a field modification of a wooden rack built and attached to the front of the basket that was not certified, approved, or authorized by the manufacturer or any nationally recognized testing laboratory exposing employees to falls, struck-by, and crushed-by hazards.

Jobsite: An employee working on the unprotected side of a five story building was not protected by guardrails, safety nets, or personal fall arrest system exposing him to a fall to the ground below. True Brothers Construction Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 933932, citation number 1, item number 1, and was affirmed as a final order on December 18, 2013, with respect to a workplace located at Lafayette Street, Salem, Massachusetts.
