29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Location:  1327 Kendon Drive, Des Peres, MO  63131  Workers were exposed to a struck-by hazard as they failed to don hard hats while picking up tear-off debris from the ground directly underneath the roofing crew.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  Location:  1327 Kendon Drive, Des Peres, MO  63131  Employees failed to wear eye protection while operating nail guns to perform roofing operations.  The Quality Home Improvements was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 316235027, citation number1, item number 1 and was affirmed as a final order on March 2, 2012, with respect to a workplace located at 9967 Canterleigh Court, Afton, MO  63123.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Location:  1327 Kendon Drive, Des Peres, MO  63131  Employees were performing residential roofing activities without the use of any fall protection system:  a) Fall hazard from the approximate 5:12 pitch main roof with an eave to ground distance of approximately 8 feet, 8 inches at the ladder landing;  b) Fall hazard from the approximate flat porch roof with an eave to ground distance of approximately 8 feet.  The Quality Home Improvements was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 316235027, citation number 1, item number 2 and was affirmed as a final order on March 2, 2012, with respect to a workplace located at 9967 Canterleigh Court, Afton, MO  63123.
