29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): A.) Fairview Manufacturing Corporation, 2505 Avonia Rd, Fairview, PA 16415, on or about July 31, 2014 and this prior thereto: Employees were performing service and maintenance activities on machinery including, but not limited to, a Fushion 250 Mazak Turning Center. The servicing and maintenance activities on the equipment included, but were not limited to, electrical troubleshooting. The lockout/tagout procedures for the machinery were missing key elements, including, but not limited to, specific language/techniques to verify deenergization, location / specific identification of isolation devices, and specific techniques for bleeding off air pressure.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A.) Fairview Manufacturing Corporation, 2505 Avonia Rd, Fairview, PA 16415, milling and drilling room, on or about July 31, 2014 and this prior thereto: Employees use a Delta 6" abrasive belt finishing machine (Cat #: 31-550, Serial #: 98F00690) to perform work tasks including, but not limited to, grinding down bolts. The abrasive belt finishing machine was not equipped with a side guard. The lack of a side guard exposed personnel to the rotating wheels that drive/move the abrasive belt.
