29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard: On or about July 29, 2014, and at times prior thereto, Rig #5, the leading edge of the stairway landing on the south side leading up to the shaker pit was sticking up approximately 2 to 3-inches above the floor. This condition exposed employees to a trip and fall hazards.

29 CFR 1910.23(a)(8): Every floor hole into which persons could accidentally walk was not guarded by either a standard railing with standard toeboard on all exposed sides, or a floor hole cover of standard strength and construction. While the cover was not in place, the floor hole was not constantly attended by someone or protected by a removable standard railing: On or about July 29, 2014, and at times prior thereto, Rig #5, at the Generator Trailer, a floor hole opening measuring approximately 9-feet 3-inches long and 32-inches wide was left unguarded. Employees were exposed to a fall hazard of greater than four feet.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: On or about July 29, 2014, and at times prior thereto, Rig #5, standard railing or the equivalent was not provided to protect employees from falling. Employees were exposed to a fall hazard of greater than four feet in the following instances: a) At the south end of the shaker pit, a mid-rail was missing. The width between the rails was 24-inches and the height between floor and top-rail was approximately 40-inches. A fall to the next partial lower level measured approximately 40-inches with additional 24-inches to the bottom. b) On the back end of the generator trailer next to the doghouse, there was a gap between rails. The distance between the two rails was approximately 39 inches wide with a fall hazard of approximately 5-feet 1-inch to the ground.

29 CFR 1910.23(d)(1)(iii): Stairway(s) less than 44 inches wide having both sides open, did not have one stair railing on each side: On or about July 29, 2014, and at times prior thereto, Rig #5, the stairway attached to and leading up to generator trailer/light Plant did not have railing on the left side. The stairway had four risers and the each stair was 22-inches in width. Employees were exposed to fall approximately 4-feet 10-inches from trailer floor to the ground.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points: On or about July 29, 2014, and at times prior thereto, Rig #5, at the Parts House/Tool Room, a 240V electrical cable providing power to the Ingersoll Rand Compressor (Model No. C8V254349) was pinched between the bottom of the metal frame and door. Employees were exposed electrical shock hazards.
