29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) Wall signage mounted above the overhead folding doors and also wrapping around the building: On or about August 1, 2014 Electricians attaching a cover to a sign while working from a ladder jack scaffold were exposed to falls of 20 feet without adequate protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Wall signage mounted above the overhead folding doors and also wrapping around the building: On or about August 1, 2014 The access ladder to the ladder jack scaffold only extended 1.5 feet while electricians were accessing and egressing the work area.
