29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Drill site: On or about July 29, 2014, and at times prior thereto, employees working on Aquila Rig #3. Behind the trailers, there were two orange 25-foot 12 AWG extension cords connected together and were feeding two black 50-foot 12 AWG extension cords; the label states that this is prohibited. This condition can potentially expose employees to electrical hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: Drill Site: On or about July 29, 2014, the strain relief to the water pump to the water and sewer system, located behind the crew living quarters was damaged. This condition exposed the employees to electrical hazards.
