29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Equipment located throughout the production facility; On or about July 16, 2014 and at times prior thereto, the employer exposed employees to potential hazardous energies in that employees were permitted to perform maintenance and servicing on equipment in the facility (including but not limited to the Air lay line and Melt blown line) without having a written lock-out tag-out program or having written machine specific lock-out tag-out procedures for equipment with more than one energy source.
