29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest system.  On or about July 03, 2014,  employees who were installing a roof were not provided fall protection.  The employees were exposed to a fall hazard at a height of over 6 feet to the ground below.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about July 03, 2014, employees were using an aluminum extension ladder with side rails that did not extend at least 3 feet above the edge of the roof.  The employees were exposed to a fall hazard.
