29 CFR 1910.1030(f)(3): Following a report of an exposure incident, the employer did not make immediately available to the exposed employee a confidential medical evaluation or follow-up:        (a) At the patients residence. The employee sustained a needlestick injury while she was cleaning the patient's room.  The employer did not ensure that the employee received a post-exposure evaluation and follow-up. The condition was noted on or about August 1, 2014.       NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER  IS REQUIRED TO SUBMIT ABATEMENT  DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19.

29 CFR 1910.1030(g)(2)(vii)(J): The Bloodborne pathogens training program did not contain information on the appropriate actions to take or persons to contact in an emergency involving blood or other potentially infectious materials:      a) Astra Care dba True Care -facility wide- Employee had a needlestick incident but the employer did not ensure that the information and training provided to the employees contained the appropriate actions to take or persons to contact in an emergency involving blood or other potentially infectious materials. The condition was noted on or about August 1, 2014.      NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY  DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT   CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS  VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

The condition was noted on or about August 1, 2014.    NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY  DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
