29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  a) On or about July 31, 2014, at the above addressed job site, the employer did not have a safety program that provided for frequent and regular inspections of the job site to identify hazardous conditions, for employees installing HVAC components on a second floor near an unguarded elevator shaft.  PLEASE NOTE THAT DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) On or about July 31, 2014, at the above addressed job site, employees were exposed to a fall hazard while installing air conditioning components on a second floor near an unguarded elevator shaft, without a means of conventional fall protection.   PLEASE NOTE THAT DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED
