29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  On or about April 29, 2014, the employer did not ensure employees working from a residential roof were in use of fall protection. Employees were exposed to a fall hazard of more than 6 feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.    On or about April 29, 2014, the employer did not ensure the side rails of an extension ladder being used by employees to gain access to the upper level of a building being reroofed extended at least 3 feet above the landing. Employees were exposed to a fall hazard.
