29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:   (a)  On or about July 28, 2014, Omni Restoration & Remediation, LLC, as a controlling employer, did not ensure that A&P Home Improvement, LLC,  protected their employees from falls in excess of six feet, while they were in the process of re-roofing a residential building with a 6/12 pitch roof, located at 6004-6006 Tomar Road, Sioux Falls, SD.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a)  On or about July 28, 2014, Omni Restoration & Remediation, LLC, as the controlling employer, did not ensure A&P Home Improvement, LLC,  protect their employees from falls, while using a portable ladder that did not extend at least 3 feet above the upper landing surface, located at 6004-6006 Tomar Road, Sioux Falls, SD.
