A. Elite Slate & Tile LLC worksite located at 34 Sturges Ave., Mansfield, Ohio: On or about August 1, 2014, the employer did not ensure employees were protected from head injuries while working below employees installing weatherproofing. Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or equipment.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. A. Elite Slate & Tile LLC worksite located at 34 Sturges Ave., Mansfield, Ohio: On or about August 1, 2014, the employer did not ensure employees were protected from fall hazards while performing weatherproofing activities on the 12/12 pitched roof. Employees were exposed to a fall distance of approximately 30.00 feet on the north side of the commercial structure.
