29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA From 300 or equivalent), was not completed in detail as required by the regulation: (a) On or about October 14, 2014 on the 2014 OSHA 300 Log, case numbers MR74401, MR78518 and MR78572 were found to be lacking an adequate description in column F of the OSHA 300 log. Column F was lacking a description of the injury or illness, the part of the body affected and/or the object or substance that directly injured the body or made the person ill.
