29 CFR 1910.119(j)(4)(ii): Inspection and testing procedures did not follow recognized and generally accepted good engineering practices. a) Crum Creek Water Treatment Plant, chlorine evaporator, pressure relief valve - The inspection and testing of the pressure relief valve on the chlorine evaporator did not follow good engineering practices such as the National Board Inspection Code. The National Board recommends an initial five year interval for inspection and testing for a pressure relief valve in combination with a rupture disk such as in this installation which may be modified based on previous inspection history. The employer did not have an inspection and testing program or inspection history for this pressure relief valve consistent with recognized and generally accepted good engineering practices.

Instance a) - Crum Creek Water Treatment Plant - Action Item 5, from PHA conducted on December 6, 2012, "Add intercom and telephone to new Sludge Building so personnel will hear any evacuation notice through PA system" had not been resolved in a timely manner as of 8/4/2014. Instance b) - Crum Creek Water Treatment Plant - Action Item 10, from PHA conducted on December 6, 2012, "Update the MC (maintenance work order system) scrubber PM items to include detailed task list/results (per 6/12/12 email) and annual check" had not been resolved in a timely manner as of 8/4/2014. Instance c) - Crum Creek Water Treatment Plant - Action Item 12, from PHA conducted on December 6, 2012, "Hold brief training on use of hand held Cl2 detector" had not been resolved in a timely manner as of 8/4/2014. Instance d) - Crum Creek Water Treatment Plant - Action Item 13, from PHA conducted on December 6, 2012, "Ask ESE (Environmental Services & Equipment, Inc.) to verify that they will lock out key liquid feed valves and use standard red LOTO tags to prevent an operator from reconnecting while liquid chlorine is out of service for maintenance/repair" had not been resolved in a timely manner as of 8/4/2014. Instance e) - Crum Creek Water Treatment Plant - Action Item 14, from PHA conducted December 6, 2012, "Remove old, corroded cylinders (containing sulfur dioxide) from Crum Chlorine Storage Room" had not been resolved in a timely manner as of 8/4/2014.
