An inspection was not conducted by the competent person prior to the start of work and as needed throughout the shift:      On or about 7/31/14    Location: 15 Bradford Ave, Rye, NY        a) Two employees worked in an unprotected trench that measured approximately 7' 5" deep. A competent person did not performed a daily inspection of the excavation.
