29 CFR 1910.95(g)(5): Within 6 months of an employee's first exposure at or above the action level, the employer did not establish a valid baseline audiogram against which subsequent audiograms could be compared. (a) Granite & Marble Designs, LLC, 5676 Pecos St., Unit A, Denver, CO 80221: On and before August 4, 2014 the employer did not establish baseline audiograms for employees exposed to 85 dBA and above in the workplace. All four employees monitored were over the OSHA Action Level of 85 dBA. This condition exposes employees to noise induced hearing loss. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer shall obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels.
