29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  (a) Roof, new residential construction located at 5035 Little Harbor Way, Acworth, GA 30101: Employee operating a pneumatic Hitachi (Model - NV45AB2) Coil Roofing Nail Gun firing 1-1/4-inch nails at 120 PSI was not protected from eye injuries. The employee operating this equipment was exposed to fasteners and flying metal debris as no eye protection was worn during use.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  a) Roof, new residential construction located at 5035 Little Harbor Way, Acworth, GA 30101: Employees engaged in roofing activities at eave heights of approximately 10-feet were not connected to the life-line provided. Employees working on the roof were not protected from falls to the lower level.  b) Roof, new residential construction located at 5035 Little Harbor Way, Acworth, GA 30101: Employee installing roof cap on a 12/12 pitched roof at eave heights of approximately 11'5" was not protected from falls to the lower level as the harness was not connected to the life-line provided.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Front side, new residential construction located at 5035 Little Harbor Way, Acworth, GA 30101: Employees were exposed to falls of approximately 10-feet while using a ladder to access the roof which was not extended at least 3-feet past the eave. The ladder was not extended or secured to the structure to prevent accidental movement during use.
