29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   Location: 460 Washington Street, New York, NY.   a) Employees were exposed to a fall of up to approximately 7 stories to the ground below while working and/or walking near an unprotected edge at the site; on or about 8/1/14.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.  Location: 460 Washington Street, New York, NY.   a) Employer did not provide a midrail at a section of guardrail in the 7th floor.  Employees were exposed to a fall hazard of approximately 7 stories to the ground below; on or about 8/1/14.
