29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: (a)Emerio's Framing at 1904 - 1964 Aldous Lane, Louisville, CO 80027: On and before August 2nd, 2014, the exposing employer allowed the employees to use a splitter that was missing its ground plug, with circular saws. The condition exposed employees to electrical shock hazards.

29 CFR 1926.502(b)(13): When guardrail systems were used around holes which were used as access (such as ladderways), they were not provided with a gate, or were not so offset that a person could not walk directly into the hole: (a)Emerio's Framing at 1904 - 1964 Aldous Lane, Louisville, CO 80027: On and before August 2nd, 2014, the exposing employer did not ensure that a ladder way point of access was provided with a gate, or with offset guardrails, to ensure that employees could not walk directly into the hole. The condition exposed employees to a fall hazard of approximately 7 feet 1 inch.
