29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a)Caraveo, Robert dba RC Construction at 551 Hoyt Lane, Louisville, CO 80027: On and before August 2nd, 2014, the exposing employer did not ensure that employees wore safety glasses when using or working in the vicinity of pneumatic nail guns. The condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: (a)Caraveo, Robert dba RC Construction at 551 Hoyt Lane, Louisville, CO 80027: On and before August 2nd, 2014, the exposing employer did not ensure that employees were protected from falling because of the following deficiencies with the guardrail system: 1) The top rail did not extend across the full length of the platform; 2) The guardrail system was missing its midrail; 3) The guardrail system was missing its end rails; 4) The guardrails did not support a 200 pound side load because the uprights were more than 8 feet apart, and were used to support a 2 x 4 top rail; and 5) The guardrails did not support a 200 pound side load because the horizontal prongs supporting the uprights fit into the pump jack scaffold platform, but these prongs were not pinned on the backside to prevent the prongs and uprights from being pushed out. The conditions exposed employees to a 13 foot fall hazard.
