29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:    (a)Oris Services at 950 28th Street, Boulder, CO 80023:  On and before August 2nd, 2014, the exposing employer did not ensure that employees wore chemical resistant protective clothing when acid pressure washing the brick on the south exterior wall.  Employees were exposed to Glycolic Acid and Hydrogen Chloride during pressure washing activities. The conditions exposed employees to chemical hazards.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a)Oris Services at 950 28th Street, Boulder, CO 80023:  On and before August 2nd, 2014, the exposing employer did not ensure that employees wore splash goggles when acid pressure washing the brick at the south exterior wall. Spray washing activities exposed employees to Glycolic Acid and Hydrogen Chloride.  The condition exposed employees to eye injuries from the acid washing.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system (a full body harness is required):    (a)Oris Services at 950 28th Street, Boulder, CO 80023:  On and before August 2nd, 2014, the exposing employer did not ensure that employees were using a lanyard attached to the boom or basket when using a Genie S-60X extended man-lift to acid pressure wash the brick at the south exterior wall.  The condition exposed employees to fall hazards of approximately 25 feet.
