29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment: a/ Van De Vries/E. Brunswick NJ Site: The hazard assessment to identify potential hazards temporary employees would be exposed to was not documented and certified. Employees are exposed to hazards associated with (including but not limited to) manually handling raw materials, finished product, minor cleaning operations where gloves, safety glasses and dust respirators may be required for use. Observed on/or about 08/05/2014.

29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous Class II, Division 2 locations were not intrinsically safe, or approved for the hazardous Class Division 2 location, or safe for the hazardous Class II Division 2 location: a/ Van De Vries/E. Brunswick NJ Site: Secondary Dust Collection System Room. Equipment rated Class II division 2 and the surrounding floor area was not provided with approved means to clean up residual dust that spills on the floor and equipment when full containers of dust is removed from the dust collection system. A broom was used for cleaning in lieu of the wet system that is used to clean the other areas of the site where dust would exist. Observed on/or about 08/05/2014.
